This position is for a Social Worker in short-term and long-term care nursing home setting.

We are looking for a dedicated, compassionate, and emotionally intelligent case worker to oversee and ensure that the medically related emotional and social needs of our nursing home residents are met and maintained on an individual basis. The social worker will work with residents in our nursing home aiding in the access of services and resources to meet those needs while collaborating with our interdisciplinary team.

Responsibilities:

• Interview residents and families to obtain social history
• Maintain written documentation in the resident medical record per the company's policy.
• Develop social service component of the resident's plan of care identifying specific problems, goals, and approaches.
• Involve residents and families in planning social service programs when possible
• Collaborate with interdisciplinary team daily
• Interpret social, psychological, and emotional needs of the resident, and family to the medical staff, attending physician, and other resident care team members.
• Provide solutions for social and practical environmental problems including seeking financial assistance, discharge planning, collaboration and referrals with community agencies as needed
• Provide support, education and resources to residents and families

Skills

• Proven work experience as a Social Worker
• Working knowledge of social theories and practices
• Social perceptiveness and empathy
• Ability to build and maintain professional helping relationships
• Ability to relate and communicate with diverse population and groups
• Resilience along with ability to assess situations
• Quick thinking, organized, reliable & caring
• Willing to submit to background checks
• BS degree in social work (BSW)
• LSW required
