PACT Outpatient RN

Duties

The PACT Outpatient RN's duties focus on direct patient care, care coordination, health promotion, clinical oversight, and team collaboration. The nurse plays a key role in managing patient panels, guiding support staff, and advancing an evidence-based practice environment.

• Provide evidence-based nursing care to primary care patients.
• Manage assigned patient panels and utilize clinical registries to ensure timely access and evaluation.
• Coordinate care for Veterans across the Primary Care Department and other services.
• Deliver health promotion, education, and coaching on wellness, prevention, and chronic disease management.
• Offer oversight and support to licensed vocational nurses and nursing assistants.
• Contribute to nursing orientation, competency verification, and ongoing education efforts.
• Engage in quality improvement activities and support outcome-driven practice enhancements.
• Participate in interdisciplinary care discussions, offering a professional nursing perspective.
• Share knowledge, provide constructive peer feedback, and foster a supportive learning environment.
• Promote and sustain evidence-based practice by evaluating clinical methods and questioning existing practices.
