Job Description

CarePartners of Connecticut, a health insurance company dedicated to comprehensive, coordinated care for residents of Connecticut, is seeking a Clinical Practice Team Lead Float Pool-RN to support our Healthcare & Medical Services department. In this role, you will serve as a clinical and operational leader across multiple partnered provider practices, functioning as a float resource to ensure consistent, high-quality patient care and efficient clinic workflows. Working within a culture that values collaboration, compassion, and patient-centered care, you will help bridge the gap between our health plan and network practices, supporting both direct patient care and practice-level quality and performance initiatives. As the Clinical Practice Team Lead Float Pool-RN, you will provide hands-on nursing care in various primary and specialty care settings within the CarePartners provider network while simultaneously guiding and mentoring clinical staff. You will oversee daily clinical operations when on site, supporting scheduling and patient flow, triaging patients, coordinating care transitions, administering medications and treatments, and ensuring adherence to evidence-based clinical protocols. You will act as a clinical resource for nurses and medical assistants, answering questions, troubleshooting complex patient situations, and modeling best practices in patient assessment, documentation, and communication. You will collaborate closely with practice managers, physicians, advanced practice providers, and CarePartners leadership to support performance goals related to quality, safety, patient experience, and efficiency. This includes helping practices understand and meet health plan quality metrics, closing gaps in care, supporting care coordination for high-risk or complex members, and promoting preventive services. You will monitor and support compliance with regulatory and accreditation standards, infection prevention practices, and organizational policies and procedures, taking a lead role in identifying areas for improvement and participating in performance improvement initiatives. Because this is a float pool leadership position, you will travel between practices within the CarePartners of Connecticut network as operational needs arise. You will adapt quickly to different clinical environments, EHR workflows, and team dynamics while maintaining a consistent standard of patient-centered care. You will help onboard new clinical staff in assigned locations, provide just-in-time training on clinical protocols and workflows, and serve as a liaison between practice teams and CarePartners to escalate issues, share feedback, and promote best practices. The Clinical Practice Team Lead Float Pool-RN will also participate in care management and care coordination activities, such as collaborating with case managers, social workers, and population health teams to address social determinants of health, reduce avoidable hospitalizations, and ensure smooth transitions of care. You will engage directly with patients and families to provide education on disease management, medications, and follow-up care, advocating for each patient’s needs and reinforcing CarePartners’ commitment to improving community health outcomes. This position is ideal for an experienced Registered Nurse who enjoys variety in clinical settings, is comfortable stepping into a leadership role, and is passionate about advancing coordinated, value-based care. The successful candidate will demonstrate strong clinical judgment, excellent communication skills, and the ability to build trust and effective working relationships with both practice staff and provider partners.
